THE FORM

In order for Rod to get on the Democratic primary ballot for delegate, he needs at least 125 Virginia voters to sign this form. A blank version of this form is available on the Department of Elections website, but we’ve filled in the pertinent fields for Rod Hall’s campaign below.

STEP BY STEP

Step 1

Download the form above by clicking the “Download Form” button. You can download as a PDF or text file.

Step 2

Open the downloaded form. In the “Petition Signer Statement” section, fill in your county, city, or town.

Step 3

Print your full name, full address, the date, and the last four digits of your SSN. Then sign the form! Be sure to use the address at which you’re registered to vote.
